Small Works Qualifications Application (MS WORD)

An application for the Small Works Roster (SWR) is valid for the calendar year in which it is received. A new SWR qualifications application must be submitted each year.

The application furnishes information for establishing qualification for the SWR.  In order to be eligible for placement, contractors must be able to meet the minimum insurance requirements and properly complete and execute the application.

Placement will be made by the date your completed application is received.
